a place on the Wait List
a New Patient Appointment Offer
If your child’s referral is accepted, we will add your child to the Wait List for an appointment with the doctor.
For referrals triaged as relatively less urgent, there may be a longer wait for an appointment offer.
Appointments may be offered at short notice, even when a referral is less urgent, and so it will be important to ensure that all information and consents are on file and ready.

To be on the wait list or to be offered an appointment, you, and also your child if aged 16 or more, must have completed the following forms:
To confirm a Wait List place, we require
that the demographic form is completed except for Medicare numbers.
The provision of Medicare numbers for your child and the parent who will be the Account Holder can be delayed until your acceptance of an appointment offer.
Adolescent Patients 16 to 18 years of age
Adolescent patients aged 16 years or older should provide their consent to share with parents, and their parent should consent to be their older child’s Account Holder.
Closure of the intake.
Completion of these forms, including the provision of Medicare numbers, is required for acceptance of an offered appointment. They are also required, except for the provision of Medicare numbers, for acceptance of a place on the Waiting List.
If these forms are not completed in the required time, the offer (of a Waiting List place or appointment) will be withdrawn, and the intake process for your child will then be closed.
