If your child’s referral is accepted, we will add your child to the waitlist for an appointment with the doctor.

For referrals triaged as relatively less urgent, there may be a longer wait for an appointment offer.

Appointments may be offered at short notice, even when a referral is less urgent, and so it will be important to ensure that all information and consents are on file and ready.

Please ensure

  • You have entered our email address and phone number in your contacts list and

  • Be alert to our email, phone calls and SMS

  • To be on the wait list or to be offered an appointment, you, and also your child if aged 16 or more, must have completed the following forms:

  • Provide your consent to the policies

  • Complete the Demographic Form for your child:

Adolescent Patients 16 to 18 years of age

  • Adolescent patients aged 16 years or older should provide their consent to share with parents, and their parent should consent to be their older child’s Account Holder.
