Medication Agreement Requests

To request your specialist’s approval of a Medication Agreement, please email us or use our contact form

We will send you a DECD form.

Please then complete and send this back, along with the following information we need for the Request.

We will need to know

  • Your child’s usual Paediatrician:

  • If they have any allergies that have not been advised to your paediatrician.

  • Is the Medication Agreement for

    • School Hours?

    • Camp or other out-of-school hours situations?

  • The date by which you need the Agreement approved, and if your Request is urgent. A request is urgent if the approved Agreement is required within the next 5 business days or before my doctor's next clinic day, whichever is later. Urgent provision may not be possible, despite the best efforts and additional work of the administrative staff and your doctor. Providers’ usual clinic days are listed on their webpage; however, they may be on leave.

    If urgent, one of the associated paediatricians may complete the Request on behalf of your usual doctor as a locum.  Please advise us if you do not consent to this.

Completing the DECD Form:

This DECD Form is a modified version of the South Australian DECD Medication Agreement Form. The format allows you to easily detail all the information needed for Medications.

Complete all the Guardian/Parent sections to provide all the details, including

  • Your child’s name and date of birth,

  • the school or other responsible entity or service.

  • The medication name, strength, dose and time and method of administration should be detailed, including

    • The form of the medication e.g., liquid, capsules, tablets, topical (such as cream or ointment), inhaler, injection etc.;

    • amount of the medication for each dose e.g., 1.5 tablets, 5 ml, 2 drops, etc.

    • The time specified should allow up to 30 minutes before or after the nominated time.

  • Some fields have drop-down boxes, and others auto-populate to avoid tedious repetition.

  • If more than four medications must be detailed, please submit a second form to include the additional medications (however, your doctor may wish to have an appointment if more than four medications are involved).

The Fee

The Fee for a Medication Agreement Request is $35 or $75 if the request is urgent.

Do you need your specialist’s approval for a new Medication Agreement?

A new Medication Agreement should only be requested if the medication management relevant to the Agreement has changed or your child is starting at a new school or centre.

Whenever medication changes are made at an appointment, you can ask your doctor to prepare a New Medication Agreement for School and Out of Hours, such as camp in the appointment.

Your GP may be able to approve the Medication Agreement for your child instead. 

Medication Agreements require a doctor’s approval for Schedule 8 Controlled Drugs, including such Opioids, Stimulants, Oxygen, Insulin, and regular pain relief.

A doctor's approval is not required for Medication Agreement Forms for as-needed or occasional pain relief such as Panadol or Nurofen, Melatonin, antiseizure medications (not Midazolam), Fluoxetine, asthma medications, Clonidine and other non-Schedule 8 Medications.
