Requests for forms, letters, or plans
Fees are charged to help to cover the costs of the significant work involved in processing and completing the Request, including both administrative and medical work.
An appointment may be required if the doctor determines the Request is more complex.

Seizure Management Plans & Seizure Management Plans with Midazolam ($35)
This form can be completed by a parent or guardian (without specialist paediatrician or neurology input) where the seizure is managed by standard seizure first aid and midazolam is NOT prescribed.
The specialist paediatrician, neurologist or neurology nurse consultant section must be completed where
• Midazolam has been prescribed for any seizure type (an Emergency Medication Management Plan must be completed)
• Any seizure type requires a non-standard first aid response
• The Parent or guardian requires support to complete this form (their GP can provide this support)
Seizure management plans that are modified, overwritten or illegible will NOT be used.
Please check to see if your doctor is required to complete the Seizure Management Plan as the processing of the Request is billed regardless of whether the form is provided.
Schools are not permitted to exclude children from attendance for medical reasons.
Other Letter
Requests for a Letter, form or brief report for another purpose, that are additional to letters generally provided in relation to a consultation, and that can be done without further consultation ($35 or more per document as requested at your provider’s discretion)
Examples may include requests for a support or explanatory letter, or a brief report for school, NDIS or another third party etc. Please provide details about the purpose of the form or letter and any needed information to allow your doctor to fully understand and quickly provide your request without further consultation.
Urgent Requests are billed an extra $40.
A request is urgent if you require the form, letter or plan within 5 business days or before your provider’s next clinic day, whichever is later.
Urgent provision within this time frame may not be possible, despite the best efforts and additional work of the administrative staff and your doctor. The providers' usual clinic days are listed on their webpage; however, they may be on leave. If urgent, one of the associated paediatricians may complete the Request on behalf of your usual doctor as a locum. Please advise us if you do not consent to this.
Other additional charges
Letters, forms or Plans are normally emailed. If you require these to be posted, an additional $15.00 charge applies.
An additional $35 may be billed if you require additional administrative work, time, and communications for your Request. Please allow us to manage your request as efficiently and cost-effectively as possible by carefully reading the provided information, submitting the form, and working with us through our streamlined procedure.
How to make the Request
Please use the Request Form below.
We send you an invoice.
Once the invoice is paid, your doctor then completes and sends the document to you by email.
The fee must be paid to proceed with the Request.
Please phone to make the payment, or email a copy of your bank’s remittance advice to show the payment. We will send a receipt.
If an appointment is required, your doctor, may, at their discretion adjust the appointment fee in light of the Request Fee already paid.
Your GP can normally complete these Medication Forms and Letters. Check with them to see if they are happy to do so.
