When starting a new referral

Or at least every 12 months, please, well before the appointment,

  • Review your provider’s Privacy Policy and other policies,

  • Complete the following forms

  • For patients aged less than 16, a parent should complete the forms

  • For patients aged 16 or older, both a parent and the patient should review the policies and provide their consent.

2. Ensure your child's Demographic File is complete and current: Please always advise us immediately of any changes to contact details, GP, parenting arrangements or responsibilities, privacy concerns, and send any current Court Orders.

Adolescent Patients 16 to 18 years of age

  • Adolescent patients aged 16 years or older should provide their consent to share with parents, and their parent should consent to be their older child’s Account Holder.
