Requests for copies of your child’s record of care

Most families already have copies of reports and letters.

Associated providers are encouraged and facilitated to copy their letters to parents via email. Most do this routinely when sending them to the GP or other specialists.

Other documents on file include correspondence from your GP or other specialists. Families usually have, or can acquire a copy of these from the sender. Families will normally have copies of any reports by other providers. Unless families opt out, letters, results, and reports are uploaded to your child’s myHealth account. Pathology and radiology reports can be accessed free of charge by your GP or other specialists.

Patients are routinely provided with or offered receipts and Medicare Statements. MyGov provides information about your payments and rebates.

Providing an additional copy of records or any documents is an additional service entailing costs. Fees are charged for such requests.

Once all verifications and payment are completed, a copy of the record(s) is transferred by email as a PDF attachment to the recipient's verified email address

Payment can be made by bank transfer, but please email a copy of your bank record. You can also pay over the phone. Additional fees may be invoiced after your payment of the minimum charge to cover any additional costs as explained below.

Consent requirements

The law protects a patient's privacy and requires us to obtain their consent to this sharing with a third party, such as a new GP, paediatrician, other specialist,  school or other person or entity. Parents are third parties once the patient turns 16.

Your provider or their reception team will need to verify the patient's (or, if less than 16, the parent’s) identity, consent, email address, and phone number. A meeting with a manager in person or via video may be required. Identification documentation may be required, especially if the patient has not been in contact with the practice for some time or if a parent was not previously known to the provider.

If required, identification documents needed will include at least an official photo ID (usually a driver’s licence or passport) and a Medicare card.

For patients under 16, parental or guardian consent is required.

Once a patient is 16 years of age or older, we cannot share their health information with their parents or any third party without the patient's consent.

Patients (or previous patients) aged 16 or older

  • must provide their own written consent and a personal phone number and email address in writing.

  • may need to meet with a manager to verify their identity, consent, and contact details. Identification documentation may be required.

Fees

The minimum fee for providing a copy of records to a patient's email address, where the record has previously been provided or offered to the patient (or their parents): $35.

Additional fees may be charged when additional work, contact and steps in the process are needed to ensure proper consent and privacy.

Additional fees charged:

  • $15 to send documents to third parties.

  • $20 or more, where verification of identity, legal standing, contact details, consent &/or clarification about the requested documents and such situations may include:

    • where the patient is aged 16 or more;

    • where the request is made some time after the last engagement with Prosper Paediatrics-associated providers.

    • where extra communication, checks or requests for information about the transfer are necessary; 

    • where a specific selection of documents or additional documents is requested.

  • $40 where there are or were relevant court orders about access, guardianship, power of attorney, parental responsibility, access to information, privacy concerns, parental estrangement or other medico-legal complexity.

  • Any costs for necessary legal advice about the request for records.

  • Any costs of the necessary redaction of records or provision of a summary or report.

  • $40 plus costs of paper, printing, compilation and sending for transfer by post, courier, fax or other means apart from email.

  • $35 for urgent requests for the provision of records within 19 business hours.
